For your first visit
Our clinic is managed by a Japanese owner. Furthermore, we are welcoming international patients. From friendly consultation for minor ailments or travel vaccinations to individual arrangements for special medical intervention at selected hospitals in Bangkok.
At your first visit, we suggest you make an appointment with our doctor in advance. It would help you to have a more smooth consultation.
If you require specific medication that is not available to you in Bangkok, the clinic can source your medication through its support network of pharmacies.
Preparing for your visit
Please bring your passport for identification

Procedures & Tests
1. Reception
You will be asked to confirm your passport, insurance card, first visit reception slip, health survey slip, and if necessary, fill out a claim form with the insurance company.
If you need a receipt for the company, please let us know in advance.
2. Medical consultation
A nurse will measure body temperature, blood pressure, height, weight, etc. as needed, and a doctor will give you a medical examination.
If your condition requires testing or treatment, you will continue to have a blood test, IV drip, or other treatment.
3. Prescription
BLEZ CLINIC prescribes medicine in the clinic to settle the issue immediately if patients have any inquiries about medication. Our group company ” BLEZ PHARMACY” contributes that achieves a more reasonable medicine price than another clinic.
4. Payment
We accept THB Cash, a credit card except for AMEX. Your medical insurance is available as a cashless service, otherwise, it is not necessary to settle.
Upon your arrival, fill out our medical questionnaire and submit your original passport, insurance policy card, and any referrals from any other institutions.

Please note that requesting to cancel prescribed medications at the time of payment requires reissuing documents, which will delay the checkout process.
If there are any medications you do not need, please do not hesitate to inform the doctor during your consultation.
Additionally, if you require a receipt addressed to your company rather than to yourself individually, please be sure to notify the reception desk when checking in.

At our clinic, we accept the following types of insurance:
- Overseas Travel Insurance
Please submit your insurance policy card. Our staff will look into it if cashless service is available.
Cashless services are available for most overseas travel insurance policies, with some exceptions.
Please contact your insurance provider directly to confirm if your specific policy covers your treatment. Once coverage is confirmed, we will assist you with the necessary insurance procedures. - Credit Card Travel Insurance
As it may take time to verify eligibility, please call the number on the back of your card in advance to confirm whether your insurance and cashless services are available. - Local Thai Health Insurance (Social Security is NOT accepted)
Please bring your insurance card. We will check if it can be used for your treatment. - Japanese National/Social Health Insurance
We can assist with the preparation of application forms (Overseas Medical Expenses Benefit).
As required documents and forms vary by municipality, please check with your local government office in Japan first. We will then help you complete the necessary paperwork.

Notice for Overseas Travel & Credit Card Insurance Users:
If you have your own insurance claim forms provided at the time of contract, please use them. If not, we have a variety of claim forms from major insurance companies available at the clinic.
